We are going to streamline the process which will result in patients being asked to attend a fewer number of appointments. Practice catchment area

Patients wishing to register must live within our practice area. To see if your address is within the practice area please use the Practice Area Checking Tool below.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

Although you will be registering with the practice you will be able to see the person of your choice – you can tell us about this at any time.

  1. If you need to be seen at short notice, and the person of your choice is not available, then you may see someone else
  2. When you come to register please bring your medical card with you together with full details of your previous GP
  3. Please tell us if you change your name or address
  4. Please register your new baby immediately

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.

Step 1: Download and complete new patient registration form

Our new patient registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Step 2: Upload registration form and scans of photo identity and proof of address

Please deliver to the practice the completed registration form and copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.
